What the questionnaire collects
- Contact details, so your letter reaches you by email and SMS
- Your answers to the medical questions, including health information
- Your payment details, entered in the questionnaire's payment step
Why we use it
So a doctor can review your request and, if approved, issue your signed letter and send it to you. Payment details are used to take payment, and to refund you if no letter is issued. Your answers are treated as confidential.
Who handles your information
- Jotform provides the questionnaire and payment form.
- Cloudflare hosts this website.
- Analytics and ad tags never receive your letter type or health details.
Cookies
